WAIVER AND RELEASE AGREEMENT

I, the undersigned participant and/or legal guardian of the minor participant (if applicable), hereby acknowledge and agree to the terms and conditions set forth in this Waiver and Release Agreement ("Agreement") with respect to my participation in the martial arts activities offered by Newaza Brazilian Jiu-Jitsu ("Newaza BJJ Inc").

Assumption of Risk: I understand martial arts activities involve inherent risks and dangers. I am aware that these activities may result in serious injury, paralysis, disability, disfigurement, or even death. Nevertheless, I choose to voluntarily participate in martial arts activities and assume all risks associated with such participation.

Release and Indemnification: In consideration of being allowed to participate in martial arts activities organized by Newaza BJJ Club, I hereby release, discharge, and hold harmless Newaza BJJ Club, its owners, employees, instructors, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to my participation in martial arts activities. This release includes but is not limited to, any negligence, tortuous conduct, breach of contract, breach of warranty, intentional wrongdoing, or any other act or omission on the part of Newaza BJJ Club or its representatives.

Insurance: I understand and acknowledge that Newaza BJJ Club does not provide any form of medical, disability, or other insurance coverage for participants. I am solely responsible for obtaining my own insurance coverage to protect myself from any financial consequences resulting from injuries sustained during my participation in martial arts activities.

Consent for Use of Likeness: I grant Newaza BJJ Club and its assigns, licensees, and legal representatives the irrevocable right to use my name, picture, portrait, or photograph, in all forms and media, for advertising, trade, or any other lawful purposes. I waive any right to inspect or approve the finished version(s) or written copy that may be created in connection therewith.

Governing Law and Severability: This Agreement shall be governed by and construed in accordance with the laws of [State]. If any provision of this Agreement is deemed invalid or unenforceable by a court of competent jurisdiction, the remaining provisions shall remain in full force and effect.
I have read this Waiver and Release Agreement carefully, understand its contents, and voluntarily agree to its terms and conditions. I acknowledge that by signing below, I am giving up certain legal rights.
